The application forms for the Soo Pee Wee Extravaganza Tournament are now available online. Please, use the links below to download and print the forms. The first page includes all of the general information regarding the tournament. The application and roster forms can be downloaded as 'pdf' or 'doc' files. These two forms must be included, with the payment, when applying for the tournament. NOTE: It is imperative that each team, participating in the tournament, fill out a 'Team Tournament Permit Request'. (See attached form below.) It is to be handed in to your League Director, a minimum of two weeks prior to the tournament. The SPWHL will be paying the fees for our association's teams only, but the team is responsible for submitting the form. Penalty fees could be assessed if forms are not turned in on time.
